Already a BEK employee?
Want to change your benefits coverage during the year due to a change in your life like getting married or having a baby? Go to dayforcehcm.com within 31 days of your qualified life event and complete a life event request. Otherwise, you (and your dependents) will not have a change in coverage. When coverage begins

BEK-provided Basic Life/AD&D insurance begins on the first day of the month following your 60th day of full-time employment (or 30 days of if you are a SE Division or Kelley Manufacturing employee.)

Your medical, dental, vision and supplemental life/AD&D insurance benefits also begin on the same day if you elect coverage before your 60th day of employment (or 30th day of employment for SE Division or Kelley Manufacturing employees.)

Changing benefits during the Year

The IRS doesn’t allow you to make changes during the year unless you have a qualified life event like getting married or having a baby. Go to dayforcehcm.com > Forms > Life Event Declaration to submit a life event declaration.

Adding a family member to your coverage

If you have a qualified life event and want to change your elections or add a dependent, go to dayforcehcm.com within 31 days of the event and submit a life event declaration. Otherwise, you (and/or your dependents) will not have a change in coverage and you can’t add or change coverage until the next Annual Enrollment period.

You will be asked to provide documentation that verifies your dependents are eligible for coverage. Be sure to return the requested documents by the deadline or your dependents will not have coverage. Find a list of acceptable dependent eligibility documents.

Naming a beneficiary

Go to dayforcehcm.com > Forms > Current Beneficiary Information to review, change, or name a beneficiary(ies) for your life/AD&D coverage.

You can name or change a beneficiary(ies) for your 401(k) and Profit Sharing account by logging on to empowermyretirement.com.

Paying for coverage

Payroll deductions will be withheld from your Ben E. Keith paycheck to pay for your benefits.

If you are enrolled in the BEK HSA Medical Plan, you may also contribute to a Health Savings Account on a pretax basis, if you do not have other disqualifying coverage.

If you enroll in an FSA, your pre-tax contribution will be deducted from your Ben E. Keith paycheck before taxes are calculated.

Review your paycheck to make sure that the proper deductions are being taken. Talk to your HR Manager or Office Manager if you have questions.

Medical plan surcharges

You may pay additional costs for medical coverage if you choose to cover your working spouse or if you use tobacco products.

SPOUSAL SURCHARGE
If your spouse is currently working and eligible for medical coverage through their employer, you will pay a $100 monthly charge in addition to your medical premiums.

TOBACCO SURCHARGE
You’ll pay $100 more per month for Ben E. Keith medical coverage if you use tobacco in any form —cigarettes, e-cigarettes, cigars, pipes, snuff or chewing tobacco. If you use tobacco but want to quit, call QuitLogix at 1-855-372-0040 or go to https://helpline.quitlogix.org/en-US. If you complete the 5 tobacco cessation sessions, the additional tobacco surcharge you are currently paying on the medical premium can be removed.

If you are unable to participate in the tobacco cessation program because your doctor has determined that it would be medically inadvisable, Ben E. Keith will work with you to provide another alternative for you to avoid the $100 per month surcharge.
